Digital Health Check
We developed and self‑funded an end‑to‑end Primary Care SaaS platform that brings the NHS Health Check directly into people’s homes. Designed for simplicity and accessibility, it allows eligible individuals to complete cardiovascular risk assessments remotely — removing barriers to participation and helping identify potential health issues earlier. Test results integrate seamlessly into patients’ digital health records, ensuring continuity of care. The platform has been successfully trialled with the Office for Health Improvement and Disparities (OHID) in Cornwall and Wolverhampton, marking a major step toward more proactive, data‑driven primary care.

Questionaire
The health check is offered by GP invitation via a text message containing a secure link. Each participant receives a unique login to access the digital health check, where they are guided through a series of questions aligned with QRISK3, AUDIT-C, and GPPAQ assessments.
Blood pressure and blood test
Patients are directed to their nearest pharmacy or GP practice to obtain a free blood pressure reading. Once the questionnaire is completed, the app automatically triggers a request to our partner lab, which then dispatches a home blood test kit for the patient to complete. Test results are displayed in the app for patients to review.
Promotional video
Prior to the trials, we produced a promotional video explaining what a digital health check entails and why people should participate. It also lends legitimacy to the process, countering any misconception that the SMS invitation might be a scam.
